Health beavior change

PART III.  PROGRAM IMPLEMENTATION & JOURNAL

This part of the project entails implementing the specific strategies you outlined in your plan, documenting your progress by monitoring the frequency of the behavior, and writing journal entries of your personal reactions to the behavior change process as it unfolds.

L. Implementation:  Implement the behavior change program outlined in your plan starting when you wake up on Monday April 11th and continue to implement the change for at least two weeks (until you go to sleep on Friday, April 22nd).  You may continue with the change after this period if you wish, but you only need to observe the behavior for this project through this time period.

M. Monitor the behavior frequency: During the two-week implementation phase, monitor the frequency of the target health behavior to keep track of your progress. If no progress is made after the first week, revise the plan. Be sure to keep a record of the frequency, as you will need to include it in your final paper.

N. Journal (OPTIONAL): Throughout the implementation phase of your HBC project, you should be journaling your personal thoughts, progress, and challenges. Examples of types of topics you may reflect on in your journal entries include: Your reactions to the change process as it unfolds. Ups and downs you experienced and any unexpected difficulties or obstacles that arise. Factors that made the behavior change process easier. How the implementation of this program affected other parts of your life. Changes in attitude, mood, etc. you experienced during the implementation. And any other reactions, reflections, or insights that occurred during the previous week’s behavior change efforts.  If you choose to do this, you should target a minimum of eight (8) journal entries are required however there is no specified length required.  This part of the assignment is designed for you to demonstrate you are processing the experience it as it happens.

NOTE TO THE WRITER: YOU HAVE WRITTEN THE PART I AND II , III, PLEASE FOLLOW THE SAME WAY  FOR REST OF THE PARTS. PLEASE INCLUDE THE CHART TOO. THANK YOU.

PART IV.  FINAL WRITE UP

Your task for the final paper is to evaluate the effectiveness of your health behavior change program, provide a chart of the progress made, as well as describe your reactions to the experience. Succinctly address each of the following with headings for each:

O. Evaluate the effectiveness of your program: Provide an evaluation of your program by addressing the following: Was it effective in meeting the behavior change goal you set? Indicate why it worked or didn’t work for you. How would you do it differently if you could do it over? If there is no change, how do you explain it? What were the limitations of the program/strategies you designed? What were the limitations of the underlying theory you chose to guide your program? Did the theory match the reality of putting the program into action? If there wasn’t any progress after one week, how did you modify your program and did the modifications make a difference? What does the literature say about these issues?  What is recommended? Any other evaluative comments you have may be included.

P. Chart your progress: Include a chart of the frequency of the health behavior (using the monitoring data you collected during baseline and implementation), clearly marking the one-week baseline period, and points at which the intervention started and ended.

Q. Your personal reaction: Describe your personal reactions to the behavior change, as well as any insights that you gained from participating in behavior change program, the project as a whole, or any of its components.
